You and your rheumatologist have the option to enter your data directly via the web into our data-base from March 2009 onwards. To grant data security, you will have to identify yourself to the online data-base using a login-name and a password. Your treating rheumatologist can apply for a patient user account at the SCQM.

If you prefer to enter your data on a paper questionnaire, you can ask your rheumatologist for a print-out of your personal questionnaire. You can send the filled-out forms to the SCQM, where your data will be entered into the data-base by our staff.

Each year, we will ask you and your treating physician to enter a yearly control questionnaire into the online data-base. If you are using the online data-entry, your questionnaires will be ready for you to fill as soon as your rheumatologist has opened a new yearly control visit. Again, if you prefer the paper forms, please ask your physician for your personalized print-out version of your questionnaires.

At any major change in disease activity or treatment, a short visit form should be entered.
